Our office wants to make the process of registering with Disability Services as easy and user friendly as possible. As a student with a disability, you must self-disclose if you have the desire to register with our office and receive services and accommodations. To do this, please complete the steps below:

Complete Registration Form

1. The student should complete a Disability Registration Form to the best of his or her ability.  This form must be completed before each semester that the student wishes to have accommodations.

Submit Documentation

2. The student needs to provide one of the following forms of documentation:
   • An active IEP (less than 3 years old) complete with all tests, diagnoses, and accommodations
   • OR a completed Disability Functional Limitations form, to be completed by a licensed doctor or practitioner familiar with student’s diagnoses and treatment (valid for 3 years).

Intake Interview

3. Once the student returns the correct documentation, the Coordinator of Disability Services will schedule the initial intake appointment. All forms must be returned before this process can continue.

Sign Acceptance of Services

4. At the initial intake appointment, both the student and Coordinator of Disability Services will sign the Roles and Responsibilities form, indicating that they have understood their responsibilities, the college’s responsibilities to them, and the student will indicate whether or not he/she wants information shared with any third party.

Notice of Approved Accommodations

5. One week prior to the start of classes the Office of Disability Services will send out an official email to all faculty of the student and to the student, indicating the accommodations he/she has been granted from the college.

Revisit Each Semester

6. It is important to note that the student needs report back to the Office of Disability Services each semester they are enrolled in order to re-sign the Roles and Responsibilities form and fill out portions of the Registration form to ensure the correct information is on file.

This process is in compliance with the: A.D.A. (Americans with Disabilities Act of 1990) and the Section 504, Rehabilitation Act of 1973.

ASU Mid-South recognizes that individuals with temporarily disabling conditions that are a result of injuries, surgery or short-term medical conditions may need access to services and resources similar to individuals with permanent disabilities. Examples of temporary disabilities may include, but are not limited to: broken limbs, hand injuries, short term impairments following surgery or medical treatments, and pregnancy.

Documentation Requirements
The student should complete a Disability Registration Form to the best of his or her ability.  The student also needs to provide a completed Disability Functional Limitations form, to be completed by the appropriate, licensed care provider for the temporary condition.  The student should contact the Director of Disability Services as soon as they know there is a need for support or accommodations.

 Potential Accommodations for Temporary Disabilities
Academic accommodations are approved on a case-by-case basis. Examples of these can include:
-Notetaking assistance
-Audio recorder for lectures
-Extended testing time
-Extended time on assignments
-Adapted test-taking (computer based tests/online testing format)
-Leave of absence due to pregnancy or associated medical conditions

The College DOES NOT provide personal assistance to students with temporary disabilities such as building-to-building transportation or the transport of books or other personal items.

Title IX
Title IX requires a school to excuse a student’s absences due to pregnancy and related conditions, for as long as the student’s doctor deems the absences to be medically necessary. When the student returns to school, she must be reinstated to the status she held when the leave began, which should include giving her the opportunity to make up any work missed. Accommodations are not retroactive. Students must request accommodations as far in advance as possible to allow for implementation. Waiting until the last minute jeopardizes the chances of utilizing accommodations.
