Process for Registering for Disability Services

  1. Student will self-disclose him or herself to the Office of Disability Services as having a disability and with a desire to register with the office to receive services and accommodations.
  2. Student will gather one of the following forms of documentation:
    1. An Active IEP (less than 3 years old) complete with all tests, diagnoses, and accommodations.
    2. A “Report of Functional Limitations” form, attached here, Functional Limitations Form to be filled out by a licensed doctor or practitioner familiar with student’s diagnoses and treatment.
  3. Student will fill out the Disability Services Registration Packet to the best of his or her ability, attached here: Disability Services Registration Packet
  4. Once the student returns with the correct documentation, the Coordinator of Disability Services will meet with the student to discuss and complete the following:
    1. What the student can expect from Disability Services
    2. What Disability Services expects from the student
    3. An outline of the differences between high school and college (if applicable)
    4. A review and discussion of requested accommodations, where it will be determined which accommodations are considered “reasonable” by the college and will be granted to the student.
    5. Both the student and Coordinator of Disability Services will sign the Roles and Responsibilities form, indicating that they have understood their responsibilities, the college’s responsibilities to them, and the student will indicate whether or not he/she wants information shared with any third party.
  5. The Office of Disability Services will send out an official email to all faculty of the student and to the student, indicating the accommodations he/she has been granted from the college.
  6. The student will report back to the Office of Disability Services each semester he/she is enrolled in order to re-sign the Roles and Responsibilities form and fill out portions of the Registration form to ensure the correct information is on file.
  7. The student will turn in a new “Report of Functional Limitations” every 3 years to ensure the correct information is on file for the student.

Section 504, Rehabilitation Act of 1973
